From “Your Plan” please select “PPO/EPO”.
From “Your Network” please select “Tri-State”.

HealthNet

Please select “Advantage Platinum” under the network choices where indicated.

If you are looking for a Health Net Mental Health Provider, you must call the MHN Network at 800-238-2719 to verify your doctor's participation in the MHN Network. If you receive services from a doctor that does not participate in the MHN Network, your services will not be covered.

HIP

Please select "Prime" from the list of networks when you are prompted to do so.

Horizon

Please select “EPO” under the network choices where indicated. 

For providers outside of the lower 10 counties of New York and the state of New Jersey, the network is “PHCS” Network (Private Healthcare System).

PerfectHealth

Please select under the Network of Providers- click “List of GHI Hospitals” for NY hospitals participating with the PerfectHealth plans.

Please select under the Network of Providers- click www.Multiplan.com for all nationwide physicians and hospitals located outside of NY.

Guardian Managed DentalGuard

Under Resources select “Doctors/Dentists/Facilities” then click “Dental Providers”. Under DHMO/Pre-Paid Networks click “Managed DentalGuard”.

Guardian DentaGuard Preferred

Under Resources select “Doctors/Dentists/Facilities” then click “Dental Providers”. Under PPO Networks click “DentalGuard Preferred”.
